Is there a way to host this web application in SharePoint Online ?

I don't think you can migrate your ASP.NET Web Application to SharePoint Online, you have to rebuild it from the scratch in SharePoint Online.

I am trying to setup a Dynamics CRM 2016 Internet Facing Deployment. The application/SQL server is a Windows Server 2012 machine and CRM is hosted as a separate website(not the default website). Where do I install AD FS? Is it supposed to be on the Active Directory server or on the application server? Are there any other factors to be considered?
yes you can deploy your ADFS on the application server. If you look at page 10 of this deployment guide - https://www.microsoft.com/en-us/download/details.aspx?id=41701 you can see that same server deployments are supported. Although, it's a bit messy and separate server deployment is recommended.
One thing to keep in mind is that whenever you need to restart your application server (installing rollups\updates) your ADFS will go down as well, and if there are other applications using it, your users will loose access to them.
It could be any server but I would suggest to use separated server for that purpose.
I want to know if there are any projects that use DNN 7 to interact with SharePoint 2013. The SharePoint 2013 is a Provider hosted. For the setup, DNN would be installed on the same server as the App server and SharePoint 2013 on a separate server. Normally, I would create a SharePoint app that would be installed on the App server. I want to do the same using DNN as the framework with the module as the SharePoint App. Is this possible? I would assume so since SP2013 uses client model. Should I create the SharePoint App project and included it in my DNN project?

You can embed SharePoint apps in DotNetNuke by making use of a html module that embeds your app in an iframe element. But since your apps can be hosted on any web hosting stack, you can even probably create a DotNetNuke module that is a Sharepoint app.
You will have to register your app with Azure ACS and with the App Management Service of the SharePoint farm or SharePoint Online tenancy, to be able to establish communication.
Then, to access Sharepoint, you will have to pass an access token in each HTTP request that you can obtain from Microsoft Azure Access Control Service.

Purpose of App;
Download link calls a downloader that generates a custom signed build of the app and logs the download to a SharePoint list.
Site has been redesigned and migrating to SharePoint 2013, looking at a provider-hosted solution for the ASP.NET app and I was wondering if it is possible to generate a build with/on an Azure solution.

Yes is possible.
The only thing which you have remember is add in your App a clientWebPart. And configure it with url to your asp.net site.
Because when you add your app in page, you must go add how Client App Part or WebPart.
Does anyone have a detailed write up on integrating Team Foundation Server 2012 (on premise) with SharePoint 2013 (on premise), specifically where SP is setup with separate Web and App Servers.
My understanding is that the TFS extensions need to be setup on the Web and App Server but installation on the Web Server is failing.
