THE PROBLEM
After looking at 50+ StackOverflow posts and trying many permutations of my htaccess file, it does nothing still.
WHAT I HAVE TRIED
Using this website to generate my htaccess file: http://www.generateit.net/mod-rewrite/
Setting AllowOverride All in my httpd.conf file and restarting Apache.

Lives in the root directory.

Change this URL:

To this:
http://www.md1network.com/find-a-local-doctor/FL/Tampa
ADDITIONALLY
Since the actual file doing the work is: http://www.md1network.com/find-a-local-doctor/index.php, I need to be able to parse the query string with PHP. Hopefully, I will still be able to do this to get the state and city.

Your existing rule looks alright but you will need an additional external redirection rule for reverse. Put this rule before your existing rule (just below RewriteBase /).
# external redirect from actual URL to pretty one

The subfolder i want to redirect to the root folder is www.example.com/subfolder
So I need to redirect everything in that subfolder to the url adress https://www.example.com except for these two folders (which are placed in the subfolder): www.example.com/subfolder/folder1 and www.example.com/subfolder/folder2

I'd say that your first attempt looks pretty good. Just make a slight modification since you say you want to redirect to the root path, not something inside the root path:
RewriteEngine On
RewriteCond %{REQUEST_URI}!^/subfolder/folder1/
RewriteCond %{REQUEST_URI}!^/subfolder/folder2/
RewriteRule ^(.*)$ / [R=301,QSD,L]
Since you want to use a distributed configuration file (as opposed to the preferred central configuration for the http host) that rule is meant to be used inside a ".htaccess" file inside the "subfolder".
It is a good idea to start out using a R=302 temporary redirection and to only change that to a R=301 permanent redirection once everything works as intended. Also you need to make sure that you always test using a fresh anonymous browser window to prevent client side caching effects.
And you also need to make sure that such distributed configuration files are considered at all by the http server for that location (see the documentation of the AllowOverride directive).
I want to rewrite a specific file on my website to another one by using htaccess:

This is the .htaccess code i'm using based on snippets i found on the internet.
Somehow this is not working, the server is returning a 404-Not-found error.
I can't see any difference with example's that are said to work, like in

If I place file.html in the root-folder, I can view it. So the rewrite definitely is not happening.
RewriteRule does not receive leading slash. Write so:
RewriteRule ^file.html$ /dir/file.html [L]
I'm attempting to redirect URLs in .htaccess that have multiple parameters for example:
http://www.example.com/index.php?p=product&id=10&parent=0
to a clean URL on another domain like
http://example2.com/product/product10/
I believe my normal method of redirection (using a PHP header in an index file of a directory I've created) will not work due to not being able to put ? in a directory name.
I've done some minor .htaccess but have no experience with escaping parameters or anything and the only tutorials I can find are for escaping only a single parameter.
Can anybody give me a few pointers please?
You can use that in your root .htaccess:

Change [R=302] for [R=301] when test work well.

5) Redirecting the domain to a new subfolder of inside public_html.

Alternatively in Apache you can rewrite this
any.domain.com/pages/32/login
or this:
any.domain.com/32/login
or even this:
any.domain.com/some-slug/32/login
to this:
any.domain.com/pages/login.php?page=32
One way or another to do this with only apache you need to supply the page id in some fashion. Keep in mind even with format any.domain.com/some-slug/32/login the content of the slug is irrelevant and won't necessarily link to the correct page. Which I imagine is undesirable and bad for SEO.
Another alternative is using RewriteMap. But this will be tricky and require reloading apache configurations whenever a page/slug is created/edit.
I understand that pages and login are static in this case and some-page-slug is changing. And you always want to redirect to static page /pages/login.php?page=32
So this is how to do it:

So I have photographs.php, and there are also sections where it will be rewritten as photographs.php?cat=somecategory
Question is, in .htaccess how do I get these both to work as such
/photographs
and
/photographs/somecategory
so that the $_GET['cat'] variable will be = somecategory
/photographs/(.+) will need to redirect to photographs.php?cat=$1, while /photographs/? will just redirect to photographs.php. if you want to be clever, you can combine it and do it in one line and /photograph will just go to photographs.php?cat=[blank].
First of all you must have the mod_rewrite module activated.
Here's what you need to add to your .htaccess file:

RewriteRule ^photographs$ photographs.php [NC]
RewriteRule ^photographs/(.*)$ photographs.php?cat=$1 [NC]
