Looks like some old version of MySQL Workbench to me. The new version creates more elegant pictures.
P.S. There are no actual foreign keys in WordPress, since they're based on MyISAM tables, so I guess those lines were drawn in Paint or smth :D

No doubt Dia is a great tool in Linux. All linux users might have come across it. It is my best friend all the time when I want to design UML ERD or Database etc.
I wonder if it has any add on or extension that can help me in GUI/Web application prototyping. Any idea??

I'm trying to find some good reads to clarify this but I'm unable to find it (or maybe I just don't know how to search this properly).
What I'm trying to find is, considering that you have an application that:
interacts with a db;
interacts with Mainframe;
interacts with external providers;
etc...
What is the best approach to draw a diagram that represents this in a functional view rather then in a more techinical view? What kind of books, articles, websites can help me to get a clearer understanding of which approach should I follow?

First thing that you should ask yourself:
Why I am drawing or want to draw a diagram about this?
Maybe you answer is :
To understand important/significiant communication between those elements.
Then we can say:
Oh maybe sequence diagram may help...
Buy may be you answer is:
Oh I am worry about topology of system components. How those things
will be deployed?
Then we can say that:
Oh, may be deployment diagram will help you...Draw one.
So depends on "context"...
UML diagrams will not solve your problems. They help you to understand tricky and hard part of your system and think about alternative solutions by visual modeling.
Modeling is not "self masturbating" activity. It is like "group sex". You will get most benefit when you do with others...
So the main problem is not to draw which diagram... The main problem is:
What kind of problem you have? With whom you wanto to find solution? What kind of benefit may you get from drawing a diagram?
For UML i definelty suggest Larman book: Applying UML and Patterns :Check at amazon
And lastly, if UML does not fit your purpose, be creative and pragmatic...Use textual desription or even create your "own" visual modeling :-) or do anthing which help you...
I was wondering how you draw diagrams in Unified Modeling Language for your project design? By hand on paper or some editors on computer? It will be great if there are some convenient editors or other gadgets for help. I am programming under Windows and Ubuntu.

I used Sparx Enterprise Architect.
I reverse the java project and then just drag and drop from the Package Explorer to the diagram. When I have finished to present the existing classes I then add UML classes in the diagram. No code generation or model driven, just UML graphical representation mixing existing classes and abstract classes. I give my diagrams to the team and let them integrate it.
I am very lazy when modeling :-)

I can see my classes and navigate from one package to another showing associations, inheritances and dependencies dynamically. No need to create a diagram per view, I prefer to use the show hide links and directly navigate in the java code and model from the same diagram it is easier and a lot more powerful because interactive.

I used Poseidon for UML for quite a while.. not that bad, it's just a little bit slow since it's written in Java.
If I remember correctly it also has a community license that can be obtained for free..
I would vote for StarUML
maybe its not brand new, but it does the job!

I use Dia for my diagramming needs. It's open source and cross platform so you can switch between OS's without worrying about which format your diagrams were in. It exports to several different image types and has a Portable version (for Windows) as well.
I have watched this keynote that talk about develop an app based on an UML model, but I can't find any tutorial or docs about it.

Have a look at ArchGenXML.
ArchGenXML is a code generator. It builds on architecture-centric, model-based and test-driven development. Generates Python code from UML-Models (XMI-Format). It's optimized for the generation of code based on the Archetypes framework.
Documentation about using it with Plone (Zope) can be found here.
As Aussie says, ArchGenXML is the way to go - I use it a lot. But use Paster and Zopeskel too. Create an "egg" skeleton as documented in the link (use the "archetype" template), then create your model (A .zargo file) in the main code directory (ie, the directory containing config.py).
This way ArchgenXML (AGX) will create everything as appropriate for an egg.

I wanted to know what tools (preferably free) can be used to layout a collection of objects and their relationship - simple relationship like members referring each other, inheritance, attributes et.al. I would prefer a tool with which I can create and update the diagram as quickly as possible.
I'm not familiar with UML, but it's probably what I'm asking about.

You also have StarUML, wich is open source and similar to Rational Rose, only a bit simpler.
ArgoUML is a good free tool.
I really like Dia. It even has a Portable edition!
It's small, fast, cross OS, simple to use, and has a lot of built in elements (specially for UML modelling).
The only problem I can foresee for you is that it follows The GIMP's GUI design (separated window for the toolbox). I happen to like it, but many people hate it.
